New default theme for forms, surveys, and quizzes

New forms, surveys, and quizzes now feature a completely redesigned default theme with modernized typography, revised spacing, and enhanced visual consistency across all components. Several elements now have new options and improved states, including payment, upload, date, and signature fields. Existing forms retain their current theme until they are republished, while new forms automatically use this new design.
