[FORMS] Add multiple terms and conditions to your forms and surveys

Forms, surveys and quizzes can now add multiple Terms & Conditions fields, just as in forms. This enhancement unifies the experience across all builders and facilitates the management of multiple consents (privacy, data, marketing, etc.).
